Terms and Conditions of your Medicus Card Membership

The Terms and Conditions stated herein, serves as the guidelines of the issuance Medicus Card and, defines the rights and obligations of the Member (“Cardholder”) and Medicus Philippines Inc. (” Medicus”). Approval of the application signifies the member’s acceptance and understanding to any and all of the Terms, Conditions, Guidelines and Rules thereof upon signing the applicable form.

A. Your Membership

Member Qualification

    • Aged 1 to 70 years old.
    • If minor, the applicant should attach parent consent to the membership form.

Membership Duration

Membership and Medicus Card benefits are valid from the date of issuance until its date of expiration, as stated on the card, unless  voluntarily cancelled by the member or terminated by Medicus Philippines Inc. Medicus Card is for personal use and is non-transferrable. Medicus Philippines Inc., at its sole discretion, may terminate the cardholder’s benefits and privileges at any given time with or without cause, and without the need for prior notice.

Member must renew the Medicus Card before expiration in order to continue enjoying the Medicus Card benefits and privileges.

B. Exclusive Privileges

Clinical Laboratory and Other Diagnostic Examination Discounts and Benefits:

You are entitled to exclusive privileges throughout the duration of your membership:

    • Clinical Laboratory Examination: 20% Discount
    • X-Ray and ECG: 15% Discount
    • Ultrasound: 10% Discount

Discounts are not valid in conjunction with Senior Citizen/ Person with Disability (PWDs) discount, Medicus promotions, packages and privileges. The Senior Citizen or PWD can avail of their SC/ PWD discounts or the Medicus Card discounts, whichever is higher.

All diagnostic examination discounts can be availed at ANY Medicus branch nationwide.

Unlimited Primary Care Physician Consultation (PCP)

PCP consultation may be conducted at any of the following centers:
Medicus Main (Quezon St.), Medicus Jaro, Medicus La Paz, Medicus Molo and Medicus Antique.

Present your Medicus Card with a valid ID to the cashier to avail of UNLIMITED medical consultations with Medicus PCP.
Patients bearing non-Medicus diagnostic examination results will not be honored.

Preferential Discounts/ Privileges with Accredited Medicus Card Partners

The cardholder is entitled to preferential discounts for purchases made at accredited business establishments (“Medicus Card Partners”) upon presentation of Medicus Card. Medicus Card Partners may require a valid ID with photo and signature to avail the privileges. Medicus Philippines Inc. is not liable for any discount or privilege not being available or being withdrawn for any reason whatsoever by any of its Medicus Card Partners.

Availment of Benefits

Medicus Card must be presented with a valid ID to the cashier upon payment to avail of the free medical consultation with Medicus Primary Care Physicians (PCP) and diagnostic examination discounts.

The Annual Physical Examination (APE) will be conducted at the designated Medicus branch. APE (CBC, Chest X-Ray (PA view), Urinalysis, Fecalysis, Physical Examination).

These Terms and Conditions shall be governed by the laws of the Republic of the Philippines.

I confirm that the information given by me is true and correct. I understand that falsifying any of the information stated herein is a sufficient ground for rejection of my application and/ or legal actions.  Having my signature at the back of the card, signify my agreement to the Medicus Card terms and conditions. Medicus Philippines Inc. reserves the right to modify the terms and conditions without prior notice to the cardholder. In the event of disputes relating to the terms and conditions, the interpretation of Medicus shall prevail and remain binding and conclusive.
